Your First Visit To the Conway Clinic for Sports Injury
Let us help you get through the initial steps of completing your paperwork so that Dr Conway can get you start working on your case that is fast and easy. You can fill out your paper work at home by printing out the paper work online. Bring them with you into the office and our staff will get you through the intial set up phase as quickly as possible.You can also wait to fill out your paper work at the office...what ever works best for you.We will guide you through each step of our 5 step process of becoming a new patient.

Patient Forms & What to Bring There are new patient intake forms available for you to download from this website. Please take the time to fill the forms out as completely as possible. You may fax or email your history into us beforehand or simply bring them with you to your initial appointment. If you are unable complete the forms in advance, we ask you to arrive to the clinic 15-20 minutes before your scheduled appointment time to complete the necessary forms. You can expect your initial visit to last anywhere from 45 minutes to an hour. When you come for your first visit please bring the following: - Your Insurance Card We will need basic information to set you up as a patient in our office. You can print your paper work out directly from our web site if you would like. Then bring that paper work in with you which will speed up the initial process. New Patient Forms marked Required under the New Patient tab on the homepage (or click on the link). You can wait to fill out your paper work at the office if you feel more comfortable with that. As a courtesy to you, we will also call your insurance company to check on your chiropractic benefits. *Please understand that every insurance plan is different, and when we check on your benefits we are doing this as a guideline for you. We highly recommend that you review your insurance before you come into the office so that you are fully aware of your benefits.

Other Diagnostic Exams In some cases the doctor will need additional information and may need to order additional tests such as blood chemistry, lab tests, x-ray, MRI, or electrodiagnostic testing. This way Dr. Conway can get a full understanding of your unique conditions before he starts. Many times these tests are unnecessary and Dr. Conway won't order them unless truly needed. Based on this information provided by these tests Dr. Conway will be better able to determine if we can help your condition or you need to be referred to another specialist. A specific treatment plan will be designed for you and will include the type of treatment and length or number of treatments you may need. |

Our daughter has been doing competitive gymnastics for the past 5 years, practicing for up to 20 hours per week. Close to 2 years ago, she badly sprained her ankle and worked with the injury for close to a year before getting referred to the Conway Clinic. While skeptical of driving over an hour for a doctor appointment, we decided to give it a try after depleting other remedies with no luck (rest, ice, STIM machines). After providing 10 sessions with the ARP machine, our daughter was back tumbling in the gym with no pain at all and with no setbacks. 6 months later, she pulled her hamstring. With no hesitation, we immediately called the Conway Clinic and began therapy on the ARP machine. Once again, after the 10 sessions, she was working 100% in the gym, fully recovered. Both of us and our, now 11 year old, daughter has full faith in the clinic and she even recommends it to her teammates!
