Request your bill

To request an itemized bill, check your current balance, or get a copy of your payment history you can e-mail our billing department at billing@conwayclinic.com. When you e-mail our billing department you will receive an automatic confirmation e-mail to alert you that your e-mail was received successfully.

If you e-mail your request please include the following information:

  • Patients full name, and date of birth.
  • List what you are requesting: Current Balance, Complete Ledger, or Payment History.
  • How you would like your bill to be sent; choose from fax, e-mail, or mailed. And please include corresponding information for the delivery method you prefer. (i.e.: the fax number you want it sent to)
